Become an Appointed Agency

Are you interested in the opportunity to offer GMI commercial transportation and related coverages to your clients? If so, please provide the following:

 

• Completed and signed GMI Agency Questionnaire and Agreement
   

• A copy of your Department of Insurance agency license/s for each of the states where you plan to write business.​

  • Agency licenses only – please don’t send any individual’s agent/broker/producer licenses
    ​

• A copy of your Errors & Omissions (E&O) policy with minimum aggregate limits of $1M.

 

• Completed and signed IRS W-9 form.
