We’d love to work with you. To begin the appointment process, please submit a complete agency appointment package using the requirements outlined below.
All materials must be complete and current to be reviewed.

Before sending your appointment request, please make sure you have all of the following:

Fully completed, signed, and dated

​Agency’s Department of Insurance Licenses
Active agency or business entity licenses only
Include licenses for each state where you intend to write business

Errors & Omissions (E&O) Insurance
Declaration page or ACORD Certificate of Insurance
Minimum liability limit of $1,000,000
Coverage must be current
Must be the 2024 version
Must be completed, signed and dated in 2026
Older versions or forms dated prior to 2026 will need to be resubmitted
Before You Submit
A quick review of these common items can help avoid delays:
âž” GMI Agency Questionnaire & Agreement is completed, signed, and dated
âž” Agency/entity licenses only (no individual agent/broker/producer licenses)
âž” License is not expired
âž” E&O is not expired
âž” W-9 is the 2024 version and is completed, signed, and dated in 2026
If anything is missing or needs updating, we’ll reach out so it can be corrected.
Once all required materials listed above are received, your appointment request will enter review.
